Prescription medications have become a key part of modern weight loss treatment. Six months ago we introduced you to retatrutide, the next-generation “triple G” weight loss drug making waves in clinical trials. Now let’s put this entire landscape in context. From semaglutide and tirzepatide to compounded GLP-1 options, patients now have more choices than ever, as well as more confusion about which path is right for them. To help gain some clarity, check out this practical guide below.

How GLP-1 Treatment Changed Weight Loss

Semaglutide was the drug that changed the conversation about obesity treatment. By mimicking the GLP-1 hormone it reduces appetite, slows gastric emptying and improves blood sugar regulation. In clinical trials semaglutide produced an average weight loss of around 15–17% of body weight over 68 weeks. The amount of weight lost was often 30–50 pounds, and that amount is genuinely life-changing for many people.

Semaglutide is approved as a weight loss and type 2 diabetes treatment, and explains why it appears under different brand names. It’s available as Ozempic (primarily diabetes-labeled), Wegovy (obesity-labeled), and through compounding pharmacies. Semaglutide is typically administered once per week via subcutaneous injection.

How Tirzepatide Builds on GLP-1 Treatment

Tirzepatide added GIP receptor agonism to GLP-1, making it a dual agonist. The results were superior to semaglutide in head-to-head comparison, with an average weight loss of 20–21% of body weight. In fact, some patients lost 25% or more.

The addition of GIP action enhances metabolic effects, improving insulin sensitivity and reducing fat storage, as well as producing more significant reductions in visceral fat. For patients who did not respond adequately to semaglutide or want stronger results, tirzepatide is currently the most effective FDA-approved option available.

How GLP-3 Could Change the Weight Loss Landscape

Retatrutide adds glucagon receptor agonism to the + GIP combination. The glucagon component is the key differentiator, as it directly increases energy expenditure. This means the body burns more calories actively, not just passively through appetite reduction. Phase 3 results showed average weight loss of 28.7% at 68 weeks, the highest ever recorded in a large pharmaceutical trial. FDA approval for GLP-3 is projected for 2026–2027.

When it becomes available, GLP-3 will likely become the first choice for patients needing the most significant metabolic intervention.

Where Compounded GLP-1 Options Fit In

During the shortage of branded GLP-1 medications, compounding pharmacies stepped in to provide semaglutide and tirzepatide at a more accessible price point. Compounded versions allow for flexible dosing, combination with other agents (like B12 or L-carnitine), and can be significantly more affordable than branded options.

It’s vital that compounded GLP-1 medications come from an accredited, 503B-compliant compounding pharmacy, and are prescribed and monitored by a qualified provider. Source and oversight matter enormously – in short, not all compounded GLP options are created equal. Our functional medicine services combine medication with nutrition guidance, lab testing and ongoing dose reviews with your provider.
